Current studies show that Peyronie's disease (plastic induration of the penis) is an inflammatory disease of the penis where the elastic layer (tunica albuginea) of the penis in the central tissue (corpora cavernosa) is injured. Consequently, the affected area loses elasticity and when filled with blood it does not stretch, but rather causes the penis to curve in the direction of the scar. Peyronie's disease is the most common cause of excessive penile curvature.
The process begins with the oxidation of various fats and protein molecules that lead to inflammation of the elastic layer (tunica albuginea) and the corpus cavernosum. This continues until finally the collagen fibers are replaced by non-elastic fibers that form the fibrous plaque (hardened nodules) very characteristic of this disease.
These plaques can be felt under the skin, usually on the top or sometimes on the sides of the penis. Most of them dissolve on their own naturally, but sometimes they persist and over time cause:
Abnormal curvature of the penis (often upwards));
- Erectile dysfunction and/or pain during erection;
- Shortening, loss of thickness and/or malformation of the penis (indentations, bottlenecks, hourglass shape);
- Sexual problems caused by anxiety and psychological problems.
Currently available treatments are not yet completely satisfactory and mostly only help to relieve the symptoms of Peyronie's disease. For less complicated cases and the initial stages there are numerous medications and compounds (most are anti-inflammatory and anti-sclerosing) that are used and tested.
If the curvature of the penis persists in the later phases of the disease, when the pain has already decreased, it is necessary to resort to surgery to correct it.
